Nodular fasciitis is a benign proliferation of fibroblasts and myofibroblasts. It frequently mistaken for sarcomatous lesion because its rapid growth, imaging features aggressive pathological findings. Though plays vital role in evaluating these lesions, strong suspicion with careful histopathology the final answer. We present patient nodular involving upper extremity. A 15-year-old boy presented complaints progressively increasing right wrist swelling since 2 months, had been having severe pain on exertion. On clinical examination was tender. MRI revealed T2 hyperintense along volar-lateral aspect wrist, completely encasing abductor pollicis longus tendon indenting extensor brevis tendon. Per operatively, tumor seen to involve sheath insinuating between tendons. Excision biopsy performed confirmed diagnosis fasciitis.
